Community Habilitation

Community Habilitation is a program that is available to people who do not live in a residence which is certified or operated by OPWDD. Community Habilitation can be delivered at any location, including the individuals home. Our supports include assistance with activities of daily living (hands-on), community inclusion, relationship building, training and support for independence in travel, transportation, adult
educational supports, development of social skills, leisure skills, self-advocacy, informed choice skills, and appropriate behavior development to help the individual access their community.

It is usual for children and adults to learn daily living and social abilities in their home with their family members. Jewel Human Services, Inc. will assist each person with learning the abilities that they need. We also assist them in integrating these abilities in the community. Our staff will work one on one with each person to assist them in achieving the greatest possible independence in numerous areas. The areas can range from mastering self-care, and household chores and not limited to acquiring independence in the community including ordering food in a restaurant and navigating
public transportation.

Location and Time: Community Habilitation can be carried out either in the individual’s home or in the community during the hours that are most convenient to the individual or their family.

JHS trained staff will work one on one with each individual helping them achieve the greatest possible independence in numerous areas ranging from mastering household chores such as washing dishes or making their bed to acquiring skills of independence within the community such as ordering food in a restaurant or navigating public transportation

The overall goal of community habilitation is to help these individuals develop skills and assist them to lead independent lives as productive members of our community.

Eligibility: Our programs are open to all adults with the developmental disabilities, including those with intellectual disability, cerebral palsy, epilepsy, autism spectrum disorders, and neurological impairments.

Cost to You: Our services are provided at no cost to Medicaid eligible individuals. This program is regulated and partially funded by OPWDD.
